Do not use this application unless you agree to protect and maintain the confidentiality of all consumer data contained herein.  Data may be shared only with those persons having permission to access these data.  All printed reports containing personal information which are produced from these data should be stored in a protected space and destroyed when their use is no longer required.  Personal data should not be viewed on a screen which is unattended or located in a public space.
